This application contains 4 pages in Microsoft Word Format, open the file and print the 4 forms.
Depending on your printer you might get an error message about the margins being outside of the printable
area. Just ignore this error, it is due to the formatting of the application forms. Just click on "Continue" or
what ever your computer uses to continue.
After you print out the application, fill out the forms per the simple instructions. On the last page, there is a map, this page is NOT required to be filled out by the applicant. The chapter president and secretary must sign this to endorse your application to the grand chapter. The section on the top is for application to the Membership at Large chapter, not the local Delta Zeta Omega chapter at UCF. By joining the Membership at Large, you lose out on local activities carried out by the chapter. So please do not sign the top section of page 4.
